Ένα ποσοστό >10% του αλκοόλ απομακρύνεται μέσω της αναπνοής, του ιδρώτα και μέσω των ούρων ενώ το 90% απομακρύνεται μέσω οξείδωσης η οποία πραγματοποιείται κυρίως στο ήπαρ. Η αιθανόλη λοιπόν διανέμεται μέσω του αίματος στους διάφορους ιστούς, με παθητική διάχυση, ανάλογα της συγκέντρωσης νερού, τον ρυθμό ροής του αίματος και τη μάζα του ιστού, μέχρι η συγκέντρωση αλκοόλ στους ιστούς να επέλθει σε ισορροπία με αυτήν του πλάσματος, το οποίο πλάσμα, ας διευκρινιστεί, ότι δεν περιέχει κάποια πρωτεΐνη δέσμευσης για το αλκοόλ. A percentage of> 10% of alcohol is eliminated through respiration, sweat and urine while 90% is eliminated through oxidation which takes place mainly in the liver. Ένα μέρος του αλκοόλ, το οποίο θα πάρει κάποιος δια της στοματικής οδού, δεν εισέρχεται στην συστηματική κυκλοφορία, αλλά, μπορεί να οξειδωθεί από το στομάχι, μέσω συγκεκριμένων ενζύμων και συγκεκριμένα των ισόμορφων της αλκοολικής αφυδρογονάσης- ADH. Η διαδικασία αυτή θεωρείται ως μεταβολισμός πρώτης διόδου και η αποτελεσματικότητα της καθορίζει την βιοδιαθεσιμότητα του αλκοόλ (Functions, 2012). Ως μεταβολισμός πρώτης διόδου θεωρείται το αποτέλεσμα του μεταβολισμού ενός φαρμάκου από το συκώτι ή το έντερο (Crabb et al., 2004).

Some of the alcohol that one takes orally does not enter the systemic circulation, but can be oxidized by the stomach through certain enzymes and specifically the isoforms of alcoholic dehydrogenase-ADH. This process is considered as first-pass metabolism and its effectiveness determines the bioavailability of alcohol (Functions, 2012). First-pass metabolism is thought to be the result of the metabolism of a drug by the liver or intestine (Crabb et al., 2004).

Η αιθανόλη απορροφάτε σχετικά αργά από το στομάχι και προχωρά ταχέως, ιδίως σε κατάσταση νηστείας, στο πρώτο μέρος του λεπτού εντέρου, στο δωδεκαδάκτυλο. Η αιθανόλη που απορροφάτε από το έντερο μεταβαίνει στο συκώτι από την πυλαία φλέβα, στο οποίο, υπάρχουν σε πολύ μεγαλύτερες ποσότητες τα ένζυμα που οξειδώνουν το αλκοόλ. Αυτά είναι το ADH, που βρίσκεται στο κυτοσόλιο, αλλά και το κυτόχρωμα P450IIE1- CYP2E1 το οποίο είναι τύπος ενζύμων που συμμετέχουν στον οξειδωτικό μεταβολισμό των φαρμάκων και βρίσκονται κυρίως στις μιτοχονδριακές μεμβράνες ή στο ενδοπλασματικό δίκτυο των κυττάρων του ήπατος. Ας τονιστεί, ότι το αλκοόλ δεν μπορεί να αποθηκευτεί από τον οργανισμό έτσι για την απομάκρυνση του από το σώμα θα πρέπει να οξειδωθεί από το ήπαρ (Crabb et al., 2004; Functions, 2012).

Ethanol is absorbed relatively slowly by the stomach and progresses rapidly, especially in the fasting state, in the first part of the small intestine, the duodenum. The ethanol that you absorb from the gut goes to the liver from the portal vein, in which there are much larger amounts of enzymes that oxidize alcohol. These are the ADH, which is found in the cytosol, but also cytochrome P450IIE1- CYP2E1 - type of enzymes involved in the oxidative metabolism of drugs and are found mainly in the mitochondrial membranes or in the endoplasmic reticulum of liver cells. It should be noted that alcohol can not be stored in the body so it must be oxidized by the liver (Crabb et al., 2004; Functions, 2012).

Το ADH απαιτεί το συμπαράγοντα NAD, για το μεταβολισμό της αιθανόλης και το αποτέλεσμα είναι η παραγωγή ακεταλδεΰδης και NADH. Η ακεταλδεΰδη οξειδώνεται περαιτέρω σε οξικό οξύ, μέσω της αλδεϋδικής αφυδρογονάσης- ALDH, ένζυμο που επίσης χρειάζεται το συμπαράγοντα NAD, έτσι προκύπτουν τελικά αυξημένες ποσότητες NADH. Απελευθερώνεται λοιπόν οξικό οξύ από το ήπαρ και μεταβολίζεται από την καρδιά και τους μυς. Από το οξικό οξύ και αναλόγως της διατροφικής, ορμονικής και της κατάστασης ενέργειας, μπορεί να προκύψει CO2, λιπαρά οξέα, κετονοσώματα και χοληστερόλη. Η μεγάλη συγκέντρωση NADH που προκύπτει, διαταράσσει την ισορροπία NAD+/NADH με αποτέλεσμα να αναστέλλονται διεργασίες όπως η γλυκόλυση, η γλυκονεογένεση, η οξείδωση λιπαρών οξέων, ο κύκλος του κιτρικού οξέος και η δράση της πυρουβικής αφυδρογονάσης. Η αναστολή της οξείδωσης και η επαγωγή της σύνθεσης λιπαρών οξέων στο ήπαρ μπορεί να οδηγήσει σε συσσώρευση τριακυλογλυκερολών που οδηγεί σε λιπώδες ήπαρ. Ακόμα, η αναστολή του κύκλου του κιτρικού οξέος οδηγεί σε συσσώρευση ακεταλδεΰδης στο ήπαρ που αντιδρά και καταστρέφει πρωτεΐνες του ήπατος. ADH requires the cofactor NAD, for the metabolism of ethanol and the result is the production of acetaldehyde and NADH. Acetaldehyde is further oxidized to acetic acid by aldehyde dehydrogenase-ALDH, an enzyme that also needs the NAD cofactor, resulting in increased amounts of NADH. Acetic acid is therefore released by the liver and metabolized by the heart and muscles. From acetic acid and depending on the nutritional, hormonal and energy status, CO2, fatty acids, ketone bodies and cholesterol can be obtained. The resulting high concentration of NADH disturbs the NAD + / NADH balance resulting in inhibition of processes such as glycolysis, gluconeogenesis, fatty acid oxidation, the citric acid cycle and the action of pyruvate dehydrogenase. Inhibition of oxidation and induction of fatty acid synthesis in the liver can lead to accumulation of triacyloglycerols leading to fatty liver. In addition, inhibition of the citric acid cycle leads to the accumulation of acetaldehyde in the liver which reacts and destroys liver proteins. The oxidation of alcohol by cytochrome P450IIE1 is considered a secondary pathway mainly for high concentrations of alcohol and appears to generate oxygen free radicals that are destructive to biological molecules. In addition, it is blamed for the interactions of alcohol with various drugs (Functions, 2012; Rocco et al., 2014).

Η αναστολή της γλυκονεογένεσης, όπως διατυπώθηκε πιο πάνω, μπορεί να οδηγήσει σε υπογλυκαιμία, επίσης, υπάρχει ανάγκη για φρουκτόζη που βοηθά στην επανάκτηση NAD (Functions, 2012). Inhibition of gluconeogenesis, as stated above, can lead to hypoglycemia, and there is a need for fructose to aid in NAD recovery (Functions, 2012). It is interesting to ask whether these events can promote increased consumption of sweets after alcohol consumption.

Η μέση μεταβολική ικανότητα απομάκρυνσης της αιθανόλης από το σώμα κυμαίνεται από 170-240gr/d σε ένα μέσο άτομο 70kg, δηλαδή περίπου 7gr/hr που ισοδυναμεί περίπου με ένα ποτό ανά ώρα. Φαίνεται ότι η μέση κατανάλωση σε οινόπνευμα είναι 200-300 gr/d για έναν αλκοολικό το οποίο αντιστοιχεί με 1400-2100 kcals, με αποτέλεσμα συνήθως να μειώνεται σημαντικά η πρόσληψη θρεπτικών (Functions, 2012).

The average metabolic capacity of ethanol to be removed from the body ranges from 170-240gr/d in an average person of 70kg, i.e. about 7gr/ hr which is equivalent to about one drink per hour. It seems that the average consumption of alcohol is 200-300gr/d for an alcoholic which corresponds to 1400-2100 kcals, with the result that usually the nutrient intake is significantly reduced (Functions, 2012).

Πολλοί αλκοολικοί πάσχουν από υποθρεψία, είτε επειδή λαμβάνουν μειωμένα απαραίτητα θρεπτικά συστατικά- πρωτογενής υποθρεψία, όπως μακροθρεπτικά και βιταμίνες, είτε επειδή το αλκοόλ και ο μεταβολισμός του εμποδίζουν της κανονική απορρόφηση, πέψη και χρήση αυτών των θρεπτικών- δευτερογενής υποθρεψία. Ως εκ τούτου, σε πολλές περιπτώσεις οι αλκοολικοί έχουν ορισμένες ανεπάρκειες, ιδιαίτερα σε πρωτεΐνες και βιταμίνη Α με αρνητικές συνέπειες στην υγεία του ήπατος. Επιπλέον, κάποια από τα προϊόντα του μεταβολισμού της αιθανόλης μπορούν να επέμβουν στο φυσιολογικό μεταβολισμό, κυρίως των λιπών και να αυξήσουν το κίνδυνο καταστροφής ηπατικών κυττάρων (Lieber, 2003).

Many alcoholics suffer from malnutrition, either because they receive reduced essential nutrients - primary malnutrition, such as macronutrients and vitamins, or because alcohol and its metabolism interfere with the normal absorption, digestion and use of these nutrients - secondary malnutrition. Therefore, in many cases alcoholics have certain deficiencies, especially in protein and vitamin A with negative effects on liver health. In addition, some of the products of ethanol metabolism may interfere with normal metabolism, especially of fats, and increase the risk of liver cell damage (Lieber, 2003).

Φυσικά υπάρχουν και τα άτομα που καταναλώνουν μεγάλες ποσότητες αλκοόλ και δεν χρειάζονται ιατρική φροντίδα, τα οποία δεν παρουσιάζουν υποθρεψία. Αντίθετα, ίσως έχουν αυξημένο σωματικό βάρος ιδίως αν συνδυάζουν το αλκοόλ με πλούσια σε λιπαρά δίαιτα και μειωμένη φυσική δραστηριότητα. Συνήθως, μέτρια κατανάλωση αλκοόλ, περίπου στο 16%-23% των συνολικών ημερήσιων θερμίδων οδηγεί σε μικρή αύξηση της ενεργειακής πρόσληψης όπου η ενέργεια από το αλκοόλ αντικαθιστά τους υδατάνθρακες. Άτομα που λαμβάνουν >30% των συνολικών ημερήσιων θερμίδων σε αλκοόλ φαίνεται να έχουν σημαντικά μειωμένη πρόσληψη τόσο σε υδατάνθρακες όσο και σε λίπος και πρωτεΐνες. Επιπρόσθετα, η πρόσληψη σε βιταμίνη Α, C και θιαμίνη-Β1 ίσως είναι ανεπαρκής (Lieber, 2003).

Of course, there are also people who consume large amounts of alcohol and do not need medical care, who are not malnourished. Instead, they may gain weight, especially if they combine alcohol with a high-fat diet and reduced physical activity. Usually, moderate alcohol consumption, about 16% -23% of total daily calories leads to a small increase in energy intake where energy from alcohol replaces carbohydrates. People who consume> 30% of their total daily calories in alcohol appear to have significantly reduced intake of both carbohydrates and fats and proteins. In addition, intake of vitamin A, C and thiamine-B1 may be insufficient (Lieber, 2003).

Όσον αφορά τη δευτερογενής υποθρεψία οφείλεται είτε επειδή το αλκοόλ και ο μεταβολισμός του εμποδίζουν τη κανονική απορρόφηση είτε πέψη είτε και χρήση αυτών των θρεπτικών. Συνήθως αυτό αφορά πρωτεΐνες και βιταμίνες. Σχετικά με τις βιταμίνες, παρουσιάζονται κυρίως συμπτώματα ανεπάρκειας σε βιταμίνες Β1-θειαμίνη, Β2-Ριβοβλαβίνη, Β6-πυριδοξίνη, φολικό οξύ και βιταμίνη Α και C(Lieber, 2003)

As for secondary malnutrition, it is due either because alcohol and its metabolism prevent the normal absorption or digestion or use of these nutrients. This usually involves proteins and vitamins. Regarding vitamins, there are mainly symptoms of deficiency in vitamins B1-thiamine, B2-Ribovlavin, B6-pyridoxine, folic acid and vitamins A and C (Lieber, 2003).

Εν κατακλείδι, το αλκοόλ αποτελεί ένα θερμιδογόνο συστατικό το οποίο μεταβολίζεται κυρίως στο συκώτι από το ADH αλλά και το κυτόχρωμα CYP2E1 ιδίως αν οι ποσότητες του είναι σε μεγάλα επίπεδα. Πέραν των αλληλεπιδράσεων με πληθώρα φαρμάκων, το αλκοόλ μπορεί να αλληλοεπιδράσει και με τα ίδια τα τρόφιμα προκαλώντας ως επί το πλείστων πρωτογενής ή δευτερογενής υποθρεψίες με βασικότερο ίσως πρόβλημα την ανεπάρκεια των αλκοολικών σε βιταμίνη Α αλλά και την αυξημένη τοξικότητα αυτής στα συγκεκριμένα άτομα.

In conclusion, alcohol is a calorific component that is metabolized mainly in the liver by ADH but also cytochrome CYP2E1 especially if its amounts are in large levels. In addition to the interactions with a variety of drugs, alcohol can also interact with food itself, causing mostly primary or secondary malnutrition with perhaps the main problem being the lack of vitamin A and its increased toxicity in alcoholics.
